Consent script library
The words clinicians use to ask for agreement before Chronix Scribe listens to a consultation. The app shows the right script on the consent screen and will not capture anything until the clinician confirms that everyone present has agreed.
Chronix Scribe is provided by Chronix Health Group Ltd (company number 16663224, registered in England and Wales, registered office 11-13 Upper High Street, Epsom, Surrey, KT17 4QY, ICO registration ZB962513).
United Kingdom: version 2026-10
Rule: everyone present (the patient and anyone with them, such as a relative, carer, chaperone or interpreter, in the room or on the call) is told and agrees before capture starts.
What the clinician says to the patient
I use a note-taking tool that listens to our conversation and helps me write up your notes. It does not keep any recording, only the written text. I will check everything before it goes in your record. Is that okay with you? You can say no at any point and I will write the notes myself.
If the patient says no, press Patient declined and write the note yourself. Nothing is recorded.
What the clinician confirms in the app
I confirm that everyone in this consultation has been told that Chronix Scribe will listen to help draft the clinical note, that no audio recording is ever kept, and that they have agreed.
Using the scripts well
- Ask before you start. Capture begins only after you tick the confirmation. If anyone says no, do not use Chronix Scribe for that consultation and write the notes yourself.
- Video and phone consultations. Read the same script aloud at the start of the call and wait for a clear yes from each person.
- Changing their mind. Anyone can withdraw at any point. Stop the capture straight away; what was captured before stays part of the draft only if the patient is content for it to be used; otherwise, delete the visit.
- Capacity. Where a person may lack capacity to agree, follow your usual procedure under the Mental Capacity Act 2005 (or the equivalent law where you practise) and do not capture if you are unsure.
- The patient is somewhere else. If the patient joins from another country, the consent screen shows the stricter of the two countries' rules; follow it.
What gets recorded
Each confirmation is recorded with who confirmed it, when, for which visit and which script version was used, never any note content. These records are kept for at least 24 months as evidence that consent was given.
